New Hope is a Nurse Practitioner-owned, small-home care model providing highly personalized 24-hour support. Because we intentionally care for a small number of residents, our team can truly know each person, their medical and personal care needs, routines, preferences, family, and what brings them comfort and joy. We combine compassionate caregiving, clinical leadership, family partnership, and a peaceful home environment to help every resident live with dignity.
New Hope Care Home combines the comfort of home with the reassurance of professional clinical oversight to support residents with dignity, safety, and respect.With only a small number of residents at a time, we are able to provide individualized attention, meaningful relationships, and care plans tailored to each resident’s unique medical, cognitive, and personal needs.
Our Family Nurse Practitioner performs a comprehensive assessment prior to admission to determine the appropriate level of care and ensure every resident receives the support they need to thrive.We specialize in caring for individuals who may need more support than traditional in-home care can provide, while offering a more intimate and personalized alternative to larger facilities. Services range from daily assistance and medication management to advanced clinical oversight for residents with complex medical conditions, cognitive decline, mobility challenges, or chronic illnesses.
